Creating a Form Folder

Create a new form, starting with the form folder.

Create a New Form

    Optional Fields and Defaults

    • Leave the following as they are: Format, Submit Button Label, Show Reset Button and Reset Button Label fields. 
    • Ensure the Mailer box and Thank You button are selected. These options are necessary to the setup of your form. 
    • CNM security policy prohibits using Plone forms for collecting student information such as Social Security Number, date of birth or CNM ID numbers.
    • Adding content in the Form Prologue and Form Epilogue fields is also optional. 

    To create a new form, follow these steps:

    1. Navigate to where you want the form folder housed on the CNM site.
    2. Select Add new... --> Form Folder from the side menu.
    3. Add a Title and Description in their applicable fields.
    4. Click the Save button, at the bottom of the page, to create the new form.

    When you create a new form, the system automatically generates: 

    • Name
    • Email
    • Request
    • Spam Check
    • Submit

    You can add, edit or delete these form fields before publishing your form.

    Editing and Publishing the Form

    Before you publish your form, you need to edit the form fields and settings. If you don't configure the mailer adapter or save data adapter, your form won't send or collect form submissions.
