Supervised Field Experience (SFE) Application

This is the initial application for Supervised Field Experience (SFE) in the CNM Teacher Licensure Program. Supervised Field Experience (EDUC 2190 or SPED 2390) is to be taken as the final class in your program of study.

The application is due by October 1 for consideration to complete SFE in the Spring. The application is due March 1 for consideration to complete SFE in the Fall. This application will close at 5 p.m. on the deadline date.

Before filling out this document, you must:

  1. Read through the SFE Application Information, Checklist and Timeline e-mail
  2. Print or save a copy of the SFE Application Checklist Timeline
  3. Download a PDF Degree Works plan for each licensure pathway in MyCNM to upload in this application
  4. Have PDF copies of all passed licensure assessments to upload in this application
  5. Have a PDF copy of your NM teaching license (if you have one) or district approval to student teach to upload in this application
  6. Review the CNM SFE Information Document

By filling out this application you agree that you have done the above and are registering your intention to take the SFE class in the following term.

Agreement #1   I have read through the SFE Application Information, Checklist and Timeline e-mail, printed or saved a copy of the SFE Application Checklist Timeline, and have briefly looked through the CNM SFE Information Document.

Agreement #2   I understand that the Supervised Field Experience class (SFE) is to be taken as the last class in the program and that preference will be given to those students who have completed all of their required coursework and teacher assessments.

Please upload a PDF from CNM Degree Works in myCNM showing coursework completion. *Degree Works will not show dual licensure pathways, please complete a different plan for each licensure area.
Agreement #3   I understand that participation in the Supervised Field Experience class (SFE) requires that students have passed all content exams (see SFE Application Checklist/Timeline for details) prior to beginning their fieldwork.

Teacher Assessments Please upload all passed teacher assessments for your licensure pathway. *The SFE Application Packet has assessment information by licensure area.
Content Exam Requirements If you have not taken or passed the appropriate content knowledge exams (NES or PRAXIS) you must complete the appropriate exam to be approved for Supervised Field Experience. Please visit the NES or PRAXIS website for the appropriate exams. If you are unclear what exams you are required to take, please contact teachered@cnm.edu.

Licensure Area(s) for SFE   Please indicate the Licensure Area(s) in which you plan to conduct your SFE. Note: To have your SFE count for more than one licensure area your placement must meet the requirements for both areas. For example, licensure in Elementary and Secondary would require a placement in 7th or 8th grade.

Please note that in order to assist you with placement, you will be required to provide our office with your Field Experience Log(s), a current resume and a Letter of Introduction that we can send to potential cooperating teachers and administrators. This Letter of Introduction should discuss you, your strengths and weaknesses, and what you are looking for in a classroom. Please submit these documents in Word format to fieldexp@cnm.edu no later than March 1 for Fall placement or October 1 for Spring Placement.

 

After you complete this form, be sure to submit the following documents:

  • Signed Agreement to Provide Placement
  • Cooperating Teacher Information Sheet
  • Signed copy of the Contract for SFE
  • Field Experience Log(s)
  • A copy of your Authorization to Student Teach or district-specific clearance.

Please turn all paperwork in at the same time. Incomplete or partial paperwork will not be accepted. Paperwork can be submitted electronically to fieldexp@cnm.edu or brought to the education program offices at CNM's main campus.
