Supervised Field Experience (SFE) Application
This is the initial application for Supervised Field Experience (SFE) in the CNM Teacher Licensure Program. Supervised Field Experience (EDUC 2190 or SPED 2390) is to be taken as the final class in your program of study.
The application is due by October 1 for consideration to complete SFE in the Spring. The application is due March 1 for consideration to complete SFE in the Fall. This application will close at 5 p.m. on the deadline date.
Before filling out this document, you must:
- Read through the SFE Application Information, Checklist and Timeline e-mail
- Print or save a copy of the SFE Application Checklist Timeline
- Download a PDF Degree Works plan for each licensure pathway in MyCNM to upload in this application
- Have PDF copies of all passed licensure assessments to upload in this application
- Have a PDF copy of your NM teaching license (if you have one) or district approval to student teach to upload in this application
- Review the CNM SFE Information Document

By filling out this application you agree that you have done the above and are registering your intention to take the SFE class in the following term.
After you complete this form, be sure to submit the following documents:
- Signed Agreement to Provide Placement
- Cooperating Teacher Information Sheet
- Signed copy of the Contract for SFE
- Field Experience Log(s)
- A copy of your Authorization to Student Teach or district-specific clearance.
Please turn all paperwork in at the same time. Incomplete or partial paperwork will not be accepted. Paperwork can be submitted electronically to fieldexp@cnm.edu or brought to the education program offices at CNM's main campus.
